- Separate credentials by context. Production, staging, and development should each have their own access controls. An agent working on a demo dataset should be technically incapable of touching production. This isn't paranoia - it's table stakes for any serious operation.
- Build friction into destructive operations. Not general friction that trains your team to click through habitually - specific friction. A command that deletes more than 100 records should require typing a confirmation string, not clicking yes. That single design choice would have prevented the 25,000-document incident entirely.
- Make operations reversible where possible. Soft deletes. Audit logs. Backup exports before cleanup runs. The ability to undo an AI action in 30 seconds is worth more than any prompt engineering you'll do.
- Clarify who owns what. The developer in this story took full responsibility, and that's admirable. But in a team context, it's not enough to have cultural norms about "reading commands carefully." You need explicit protocols about what agents are authorized to do autonomously versus what requires human initiation.
The Productivity Paradox
None of this means slow down on AI adoption. The productivity gains are real and significant. The right response to increased speed isn't to remove the accelerator - it's to build better brakes.
